  1. Claimant’s Statement
  2. Declaration of Beneficial Ownership (for Trust / Keyman Policies OR if nominee is a Non-Natural Person eg. Organisation, society etc)
  3. Consent Form for Medical Report
  4. Authorisation Form For Crediting to Singapore Bank Account
  5. Death certificate (refer to Note A below)
  6. Proof of relationship between claimant and deceased (refer to Note B below)
  7. Copy of physical NRIC(s) of claimants / beneficial owner
