1. Completed claim form
2. Physician’s statement
3. Original receipts of expenses and summary of medical expenses (receipts must be “certified true copy” by a hospital officer)
4. Any other medical documents, e.g., medical examination (if any)

1.      Your claim request, submitted together with complete supporting documents, will be processed within 3 working days.

2.   Payment for approved claims will be made within 15 days.

In case of further investigation required, the above process will take 30–90 days according to the investigation period.
