FLEXAS (Fire, Lightning, Explosion, Impact of Falling Aircraft, Smoke)

Claim form (including policy number, date of incident, location of incident, cause of incident)
Detail photographs of the accident & damaged item(s)
Detail of chronology
Technical report which explain the cause of loss, damaged item(s) and recommendation
BMKG report (for lightning)
Detail list of damaged item(s) and loss estimation

Additional documents for Specific events TSFWD (Tempest, Storm, Flood and Water Damage)

Additional documents for Theft / Burglary & SRMDCC (Strike, Riot, Malicious Damage and Civil Commotion)

Others

Building

Plan / layout of the entire building and damaged buildings
Repair quotation with same specification as the damaged item(s)
Repair invoice with same specification as the damaged item(s)
Other relevant, reasonable and appropriate documents requested by the Company (TMI) in connection with the settlement of claims

Contents

Details of damaged item(s) along with brand information, type, categorize, and year of purchase
Details all of contents which stated the amount
Initial purchase invoice
Repair/replace quotation with same specification as the damaged item(s)
Repair/replace invoice with same specification as the damaged item(s)
Other relevant, reasonable and appropriate documents requested by the Company (TMI) in connection with the settlement of claims

Machinery and Equipment

Details of damaged machinery/equipment along with brand information, type, categorize, and year of purchase
Initial purchase invoice
Technical report from repairer with explain the cause of loss, damaged item(s) and recommendation
Maintenance record 3 months before accident
List asset of all machinery / equipment
Repair/replace quotation with same specification as the damaged item(s)
Repair/replace invoice same specification as the damaged item(s)
Other relevant, reasonable and appropriate documents requested by the Company (TMI) in connection with the settlement of claims

Stock

Details of damaged stock
Details of all stock before the loss
Initial invoice of stock
Detail cost of goods sold
Stock card/mutation stock 3 months before incident
Supporting document which show the unit price claimed
Other relevant, reasonable and appropriate documents requested by the Company (TMI) in connection with the settlement of claims

Policy holders and/or the Insured are mandatory to submit all documents/information related to the claims submitted
Request of supporting documents/information depends on the type of claim submitted
Claims can only be processed if all documents have been completed
The Company (TMI) has the right to carry out further investigations on the claims submitted by Policy Holders and/or the Insured if found document and/or information that is deemed contradictory or inconsistent with the claims submitted

The above requested documents shall not be deemed as final and we reserve our right to obtain additional information/documents apart of the above if necessary
